Middle States Region Hallmark Awards-Entry Forms

An entry form is required for each entry your chapter submits.  The entry form must be the first page of an entry.  The entry forms are specific to the award name so you must print out each specific entry form.  It must be completely filled out and signed or it will be disqualified.  If an entry is received without the entry form it will be disqualified.

For the Chapter President, Chapter Officer and Chapter Member Awards, the Middle States Region entry sheet for these award entries are titled as “Hall of Honor”.  (one for each category) Up to 20 Hall of Honor winners may be chosen for each category.  From those Hall of Honor winners, the Distinguished Chapter Member, Officer and President will be selected. No additional paperwork is required.  The rubrics and entry requirements are the same for both International Hallmark Awards and Middle States Regional Hallmark Awards.  The only difference is the name of the award for Chapter Member, Officer and President. Please note that chapters are required to send TWO copies of all awards for the Middle States Region Hallmark Awards; each copy requires an entry form.

Each chapter should include ONE copy of the Middle States Region Summary Checklist with their package.  This checklist should be placed on top of the package.  Please do not use folders to separate entries.

Listed below are the awards that are available for chapters that participated in the MS Regional Program "Common Roots" The award entry forms can be downloaded by clicking here. Also available are the Wellspring Creative Award and Wellspring Essay award which can be submitted by individual members.

Regional Program Award Entries should be sent with Middle States Regional Hallmark Awards and NOT to be sent to International HQ's. These awards are presented by the Middle States Region only.
