An entry form is required for each entry your chapter submits.  The entry form must be the first page of an entry.  The entry forms are specific to the award name so you must print out each specific entry form.  It must be completely filled out and signed or it will be disqualified.  This is extremely important as this year; both the Middle States Region and International Award entries are requiring only 1 copy.  If an entry is received without the entry form it will be disqualified.

For the Chapter President, Chapter Officer and Chapter Member Awards, the Middle States Region entry sheet for these award entries are titled as “Hall of Honor”.  (one for each category) Those same award entries for Internationals is titled Distinguished Chapter President, Officer and Member.  You will submit the same entry as it is written for both Internationals and Middle States EXCEPT that you will use the Hall of Honor Entry Form for Middle States Region submission and Distinguished Chapter President, Officer and Member for Internationals.
